If your insurance requires a referral to see a specialist, it is your responsibility to obtain one prior to your appointment. If you have any questions, please check with your insurance company and/or primary care physician.
If you were treated through the Emergency Department by one of our surgeons, and your insurance is not listed above, in most cases your care with us should be covered by your insurance company. Please check with your insurance company if you have any concerns.
We can provide care "out of network" if we do not participate with your insurance company; however, it is your responsibility to obtain approval for this and we will need documentation confirming out of network coverage prior to your first visit with us.
If you do not have health insurance, there are charges that you will be responsible for at the time of your first office visit. Please check with our office when you schedule an appointment if you have questions in regards to this.
